You own your health and wellness information. You also have the power to share it with trusted providers, and allow them to enter notes and details in your portal for you, if you choose. The power and control is up to you.

Health advice
The information provided by I Am Well Foundation is for information and educational purposes, and should not be used to replace professional or licensed health, mental health, financial, legal or other professionally licensed advice.
Privacy
I Am Well Foundation does not sell, rent or share your personal health and wellness information. You may choose to share your Healthy You Dash profile with care support team members, and you can remove or edit permissions at any time. We take precautions to protect your online data, but online health information carries inherent risk.
Payment
I Am Well Foundation is a 501(c)(3) nonprofit, and we provide this service free to residents of Darke County, OH and residents anywhere in the United States. At the end of your enrollment, we’ll ask if you would like to become a member or make a donation to support our work — it’s okay to say not at this time. Membership is $10.00 per year, and gives you access to discounts, promotions or other benefits offered by our Provider Partners.

At I Am Well Foundation, we believe in the "lightest touch" philosophy — we strive to empower people to do things on their own when they can, and provide support for those who need it. This enrollment process is designed as a self-assessment. If you need support completing it, Whole Person Care Coordination Navigation is currently available for residents of Darke County, OH.
